৪ গ্রাহক ও জামিনদার উভয়ের সচল মোবাইল নম্বর
৫ গ্রাহক ও জামিনদার উভয়ের বিস্তারিত বর্তমান ও স্থায়ী ঠিকানা
৬ গ্রাহক ও জামিনদার উভয়ের অফিস আইডি ও ঠিকানা (চাকুরীজীবী হলে)/ ট্রেড লাইসেন্স এর ফটোকপি ও ঠিকানা (ব্যবসায়ী হলে)
